Participation Consent

I give permission for my child to participate in all supervised activities of the Viva Musica Summer Day Camp including outdoor play and supervised walks (approximately 10 minutes each way) to a nearby community playground. Children will remain under staff supervision and are expected to follow all camp safety rules and instructions.*

Acknowledgement of Risk

I understand that participation in camp activities involves inherent risks, including but not limited to slips, falls, playground injuries, weather conditions, and walking on public sidewalks and crossing streets. I voluntarily accept these ordinary risks on behalf of my child. *

Medical Authorization

I authorize camp staff to administer basic first aid, including cleaning minor cuts and scrapes, applying bandages, antiseptic, and ice packs. In the event of an emergency, I authorize Viva Musica staff to obtain medical treatment for my child if I cannot be reached immediately. I understand I am responsible for any resulting medical expenses. *

Sunscreen & Hydration

I authorize camp staff to encourage regular hydration, outdoor sun safety, and to assist my child with applying sunscreen provided by my family, if assistance is needed.*

Release of Liability

To the fullest extent permitted by the laws of the Province of Alberta, I release and hold harmless Viva Musica Academy of Performing Arts, its directors, employees, instructors, volunteers, and agents from claims arising from the ordinary risks associated with my child's participation in camp activities, except where caused by gross negligence, reckless conduct, or intentional misconduct.*

Parent/Guardian Declaration

I confirm that I have read and understand this consent, authorization, and waiver, and voluntarily give permission for my child to participate in the Viva Musica Summer Day Camp.
